Sage Active Clinic Conditions Of Entry
PLEASE Do Not Enter The Clinic If Your Answers Are Yes To Any Of The Following Questions
NO COVID-19
Do you have a confirmed case of COVID-19?
NO CONTACT
Have you been or are you in close contact, within the past 14 days, with a person/s who had/has a confirmed case of COVID-19.
NO COLD/FEVER/SYMPTOMS
Do you have or have you had a fever and/or acute respiratory symptoms such as shortness of breath, sore throat or cough within the past 14 days?
NO TRAVEL (in the past 14 days)
Have you returned from overseas within the past 14 days?
If your answer is yes to any of the above screening questions then you must not enter the Clinic.
If your answer is no to all of the above questions, you may enter the Clinic on the condition that you sanitise your hands before entry and agree to follow all of the hygiene procedures as set out by your practitioner agree to a Body Temperature Test.
*Coughing/Sneezing Please ensure you cover yourself adequately when coughing/sneezing by doing so into your elbow or covering your face with a tissue or face mask.
What Sage Active Clinic is doing to look after you.
We test all those that enter the Clinic with a non-contact infrared Thermometer
We have opened the doors and adjusted the air conditioning to limit door handle use and enhance airflow.
We are providing and promoting hand sanitiser use on entering the building and during treatment where appropriate.
We ensure at least 1.5 metre physical distancing where appropriate to reduce in person contact for both staff and clients.
We restrict the number of people in the clinic to no more than eight.
A face mask must be worn whilst in the building.
We clean and disinfect equipment and touch surfaces regularly.
We ensure all staff and clients comply with the Sage Active Clinic Conditions of Entry.
